New to the plan? First stop: register for an online account so you can check your benefits and claims, update your personal information and more.

  • Select Log in then choose Create account and follow the prompts
  • Print your plan ID card or email yourself a digital version

Use your plan

Get the most out of your benefits and minimize costs by visiting only providers in your exclusive provider benefit (EPB) plan's network.

  • Estimate your cost

    Before your visit, review your  benefit highlights (PDF, 83 KB) to check the copayments for covered procedures under your plan.

  • Find a dentist

    Choose a provider that's in the network for your specific plan. 

    • Find a dentist by entering your ZIP code and selecting your plan
    • Search for a provider in your area
    • Schedule an appointment

  • Visit the dentist

    • Show your plan ID card — printed or on your mobile device
    • If you don't have your plan ID card, provide your date of birth and enrollee ID number
    • Pay any copayment due for your in-network visit
    • Out-of-network visits are not covered (unless you live more than 20 miles from a network provider*) and you'll need to pay in full at the time of service

  • Review your claims

    Log in to your account and select Claims to verify past services and procedures, along with the amount you paid for each.

    Be sure that you've opted to go paperless in My profile so you get an email whenever a claim has been processed and is available for review.

* Under this plan, you must visit a dentist from your plan's Dentegra EPB network to receive benefits. If you visit a dentist outside of this network, you are responsible for the full cost of treatment. Exceptions include: If there is no Dentegra EPB network dentist within 20 miles from home or work, certain emergency and other services are listed as covered in the out-of-network services section of your Evidence of Coverage booklet.
